Tattoo Consent Form

I, Maclin Bilski, hereby grant Sophia's Tattoo Artist, the irrevocable right and permission to use, reproduce, and display the original artwork created by me ("Artwork") for the sole purpose of creating a tattoo.

1. Grant of Rights I hereby grant to the Tattoo Artist or Client a non-exclusive, royalty-free, worldwide license to use, reproduce, and display the Artwork for the purpose of creating a tattoo on their body or the body of their client. The Tattoo Artist or Client may not use the Artwork for any other purposes, including but not limited to creating merchandise, advertising, or promotional materials, without my prior written consent.

2. Ownership and Copyright I retain all copyright and ownership rights in the Artwork. The Tattoo Artist or Client acknowledges that they have no right to claim any copyright, ownership, or other intellectual property rights in the Artwork, except as specifically granted in this consent form.

3. Credit and Attribution The Tattoo Artist or Client agrees to credit me, Maclin Bilski, as the original creator of the Artwork whenever it is displayed or shared, either in person or on any social media platform or other public forum.

4. Indemnification The Tattoo Artist or Client agrees to indemnify and hold me harmless from and against any claims, damages, losses, and expenses, including reasonable attorney's fees, arising out of or related to the use, reproduction, or display of the Artwork as a tattoo, including but not limited to any claims for personal injury, property damage, or infringement of any intellectual property rights.

5. Governing Law This consent form shall be governed by and construed in accordance with the laws of the Ohio, without regard to its conflict of laws principles.

By purchasing this high resolution image of my artwork, the Tattoo Artist and Client agrees to be bound by the terms and conditions stated above.
